We are approved providers for most insurances including Blue Cross Blue Shield, United Healthcare, FirstCare, Cigna, Aetna, UMR, IMS, Magellan, Amerigroup, Molina, Superior, Medicaid and CHIP among others. We are NOT Tricare providers. We suggest that you call the customer service number on the back of your insurance card to make sure we are providers for your  particular plan and that you are aware of your coverage and benefits including co-pays and deductibles.

Your insurance is your responsibility:


To help us help you, please do the following:


1) Make sure to bring all of your insurance information  including your identification card. We cannot bill your insurance without this information.


2) If your insurance company requires you to obtain a referral from your Primary Care Physician (PCP) to be receive services, it is your responsibility to call your insurance company to find out what their requirements are and have your PCP provide a valid referral.


3) Be prepared to pay your Co-Pay, as required by your insurance company, at each time of service. Occasionally payment arrangements can be made for large co-pays and/or deductibles.


A Note About Insurance:


We are not a party to the contract between you and your insurance company — only your insurance company or your employer’s Human Resources department can answer questions regarding your benefits. Please be aware that some services provided may be “noncovered”, or may not be considered reasonable and necessary by your insurance company. We suggest that you review your policy in full so that you understand what services will be covered by insurance, and what you may be responsible for.


A Note About EAP Referral Authorization:


We need a valid authorization number from your employee assistance programs (EAP). All EAPs require a valid referral and/or authorization number at the time of service – that means at your very first appointment. It is your responsibility to obtain this if you need it before you come in. If you don’t have it, you may be responsible for the cost of your first or subsequent sessions.


No one will be turned away for an inability to pay. We can apply a sliding fee scale for those in need. At the very least, you will receive a referral to another source who can provide you and your child with quality services.

We reserve the right to send an outstanding balance to collection after reasonable attempts have been made to collect and to add collection fees to the outstanding balance.
